When reviewing a medication order, our Nurses will ensure components stated below are included in the prescription before administering the medication. If a pertinent piece of information is not included, the nurse will contact the prescribing provider to clarify and correct the order.

  • Name of the patient
  • Age or date of birth
  • Date and time of the order
  • Drug name
  • Dose, frequency, and route
  • Name/Signature of the prescriber
  • Weight of the patient to facilitate dose calculation when applicable. (Note that dose calculations are based on metric weight: kilograms for children/adults or grams for newborns)
  • Dose calculation requirements, when applicable
  • Exact strength or concentration, when applicable
  • Quantity and/or duration of the prescription, when applicable
  • Specific instructions for use, when applicable
